ADRCs provide information and assistance, long-term care options counseling, benefits counseling, help with transitions from a hospital or nursing facility and connections to services that can help maintain good health.
Long-term care services are those that you will receive for a long time. These services may be provided in your home or possibly a facility such as a nursing home. Long-term care includes, but is not limited to, help monitoring your health or taking your medication, someone to help you bathe, dress or use the bathroom.
