Applicants must have an eyeglasses prescription dated within the last 24 months, including their pupillary distance (PD) measurement.
Applications are submitted online. It is optimal for social service agencies/health advocates to apply on a client’s behalf.
However, individuals who do not have access to an agency may apply directly but must provide proof of eligibility.
Each application has a $10 non-refundable application fee for administrative costs.
If the application is approved, a unique e-voucher number is emailed with a link to our eyeglass ordering site and instructions on how to order eyeglasses.
Approved applicants must order their glasses through New Eyes' online eyeglasses dispenser at neweyesglasses.org.
