Services include but are not limited to:
- Acute and chronic illness care
- Children's wellness
- Breast cancer screenings
- Cardiac evaluations
- Comprehensive Type II diabetes management
- Respiration therapy
- Vision and hearing tests
- Lab work
- Nutrition counseling
- Specialist referrals
Must live in Harris, Montgomery or Waller County.
This program helps people with income at or below 200% of federal poverty guidelines.
Must be uninsured.
