Meals on Wheels and More provides meal services for eligible participants as well as other home assistance and repairs. Assistance is provided to persons with disabilities who are homebound and to persons who are elderly. Other services that are available include, grocery assistance, minor home repairs and evening meals for low income children.
CASE MANAGEMENT
The Supportive Case Management team conducts in-home visits at least once a year to holistically assess each client’s needs, not only in terms of nutrition and meal eligibility, but to determine whether the client may require additional support. The Supportive Case Managers work closely with our clients, their loved ones, state and local agencies, and other Central Texas Meals on Wheels programs to fulfill any unmet needs. Any needs requiring intensive attention will be referred to the Comprehensive Assessment Resources and Education team (CARE Team) for greater attention and resources.
CARE provides comprehensive case management, financial assistance, falls and fire prevention assessments, along with assistance in applying for other benefits. The ultimate goal is to provide resources necessary to help clients solve complicated problems that will help them remain stably housed and that, if left unsolved, could lead to premature institutionalization.
Individuals enrolled in any of our programs are eligible for Case Management. Referral must come through our Supportive Case Manager or from a Program Service Coordinator.
SAFETY
Silver Watch - regular check-ins for our most frail clients by the Travis County Precinct 5’s Constable’s office. Our volunteers are our eyes and ears, but some clients might want additional check-ins. With Silver Watch, we can provide those extra safety checks to our neighbors.
TECHNOLOGY
Connecting Seniors with Technology - assistance to increase client’s knowledge and comfort level with technology, allowing them to unlock the Internet and take advantage of social media, video chatting, and other educational and informational resources. Telehealth Treatment for Depression - we are pleased to share that our Telehealth program, which began as a pilot project with the Steve Hicks School of Social Work at the University of Texas at Austin, will now become a permanent program of Meals on Wheels Central Texas (MOWCTX). We look forward to enrolling new clients after conclusion of the transition period in the Spring of 2020. In the meantime, the trained mental health counselors in this program will continue to provide treatment to clients already enrolled in the program through completion.
